Enter your Member Number, First Name and Last Name as they appear on your last paper statement (Note: Your Member Number is a six digit number (add a zero in front if five digits, spouses should add a letter "A" after the six digits). Your First Name is typically in our system as your given name (and may include your middle name or middle initial with punctuation), please use initial caps.  Your Last Name should be entered with initial caps. If the information entered does not exactly match that which is present in our files, you will be unable to register.
